Injecting acne scars with fillers can be a very satisfying procedure with relatively minimal downtime. However, because acne scars may tether down the skin, a few of these indented scars cannot be smoothed out with filler alone. When combining subcision with fillers, you begin to notice an entirely satisfying experience. A treatment, considered a match made in heaven, is carefully formulated and meticulously performed to pop out and gently sculpt the region where depressed scarring is present. Subcision works by stimulating the skin to promote the secretion of collagen, which slowly compensates for the slow erosion of the dermal filler.
This process offers a promising solution for acne scars, providing an optimistic outlook for patients. When the centre portion of the indented acne scar is tethered down with collagen fibres, the filler tends to spread around this area, creating a ‘doughnut effect’. For example, the indentation barely improves, and the margins of the acne scar become filled and rise up higher, which can worsen the overall appearance.
In these cases, subcision helps untether the collagen fibres and allows the filler to elevate, filling out the middle area of the indented scar. Without subcision, the filler does not have enough “hydraulic” pressure to push out and treat the indentation.

If the scar does not lift easily, she will perform a light subcision with the same tiny needle used to inject the filler to release the collagen fibres before adding more filler. This helps avoid the “doughnut effect.”
Because the subcised area is much smaller and a smaller needle is used, the amount of bruising is greatly reduced. Furthermore, the filler that pushes out the indentation also acts to compress the subsidised area. This effectively reduces the amount of bleeding and subsequent bruising compared to a traditional subcision. This specialised technique allows patients to experience the benefits of subcision with minimal recovery time, providing a convenient and comfortable experience.
